Customer Agreement
Please print this agreement and complete the necessary information in ink on the hard copy. 
Then read, sign and send this agreement, with the payment due.

Your signature on this agreement serves as your consent to allow Give Thanks to create your designs using their best judgment and discretion based on your selections. We reserve the right to substitute fillers, greenery and crystal sizes depending on the availability at the time your order is placed. Because our greens and fillers are products of nature, there will be a slight variation in color. Due to the fact that all of our flowers are handmade, slight variations in size, shape and color will occur.

A payment plan can be set up if your wedding date is more than 6 months away. Payment in full is due if your wedding date is less than 6 months away. Only cash, certified checks and money orders are accepted. Make payment to: Give Thanks. Tax will be applied to all orders placed within Pennsylvania.

Checks returned unpaid will be subject to a $25.00 charge.

Canceled orders will be subject to the surrender of the payment and the completed designs.

We advise you to make your selections very carefully. Orders are not refundable, all sales are final.
Errors made by Give Thanks will be replaced.

All shipments are fully insured against damage. After your designs have been completed and delivered, any further changes or alterations you may desire shall be charged to you including the costs of materials, labor, shipping, handling and tax.

The laws of Pennsylvania will govern this agreement and venue for any dispute will be in Allegheny County, Pa. In any dispute, the prevailing party will be entitled to recover fees and costs.
